Outreach Care

What Outreach is:
Outreach care provides flexible, visiting support to people living in their own homes or family homes. Support workers visit at agreed times to help with daily living, routines, and independence skills.

Best suited for people who:

-Live independently (or want to) but need some support

-Are building confidence after leaving residential care

-Have learning disabilities, autism, or mental health needs

-Only require support at certain times rather than 24/7 care
